COVID Vaccine Appt.

Our district secured 100 Covid 19 vaccines and I needed a form that would allow teachers and staff to sign up with a maximum of 5 people every 15 minutes to allow for effective and timely administration of doses without long lines or extended time out of classrooms or from other job assignments. Form had to be easy enough for individuals with limited reading ability to complete with little assistance and still provide all information required.

From the time I was notified that we needed a registration form, we had less than 24 hours for form to be created, distributed and for individuals to signup before the doses had to be administered. Once Stephen realized what I was asking for, he immediately jumped on board and created exactly what the district needed and had it ready to send out the next morning and all slots were filled by noonish. The next day Stephen helped monitor the datasheet created and make minor adjustments and corrections as needed.

I am always amazed at the quality of work and the efficiency of detail that Stephen puts into his forms and other handiwork. His potential is above my wildest expectations.
